If you have recently gotten married/divorced, etc, please update our office with any new information. Here are some things to consider.
How do I change my legal name on my employment records?
Requests for legal name changes must be accompanied by official documentation of the change. Please notify Human Resource Services of your request and provide the requested following documentation:
- Social Security card showing your new legal name, AND
- Notarized copy of marriage certificate, divorce decree, or court order documenting the legal name change.
If you are on campus, you may bring the originals of the above documents to the Human Resource Services Office. We will make the legal name change and return the original document to you immediately.
Requests for legal name changes cannot be honored without the documentation above.
Additional things to consider:
- Confidential Information Form - to update any dependent information
- Residency Certification Form – please complete ONLY if you are changing your address.
- Payroll Direct Deposit form – please complete ONLY if you are changing banking information.
- – please complete ONLY if you are changing your name and/or tax withholding information.
- You may add or remove dependents to/from your insurance coverages within 31 days from the date of change. To do so, please complete the Enrollment Change Form and Life Status Change Form. If you'd like to add/change your Flexible Spending Account elections, please complete and return the HealthSmart Life Status Change Form.
